    Fill out this form to pre-qualify for an interview with Belinda. This is the first step in our adoption process and is NOT the adoption application.
    Please note:
     ​- We do not keep previous questionnaires on file
     - A new form must be submitted for each dog you are interested in
     - Only applicants who appear to be a good match will be contacted for a phone interview

    At CB Rescue, we believe the right match matters more than choosing based on appearance. Every dog is unique, and Belinda may recommend a dog that is a better fit for your lifestyle.

    Adoption Fees & Eligibility
    Adoptions fees are:
    • $850 for dogs under 8 years old
    • $550 for dogs over 8 years old.

    ​If you rent or live in a townhome or condo, you will be required to provide written confirmation from your landlord or condo board that the dog you are applying for is permitted.

    There is no “perfect” schedule — we simply value honesty so we can match the right dog to the right home.

    Important to know

    IF you are selected for a phone interview and there is a mutual agreement to move forward with the adoption application, it is important to know that there is a non-refundable $25 fee.
