Adoption Pre-qualification Questions

Fill out this form to PREQUALIFY you for an interview with Belinda. This is only the first step in our adoption process. This is NOT the adoption application. We do not keep previous questionnaires on file. We require a new one for each dog you are interested in.

We will only contact those for a phone interview who are the right fit for each dog.

Adoptions fees are: $850 for dogs under 8 years old and $550 for dogs over 8 years old.

We feed the dogs an appropriate biological diet of fruits, vegetables and raw food. Would you be open to feeding the dog this way? *

Important to know

If you are selected for a phone interview and there is a mutual agreement to move forward with the adoption application, it is important to know that there is a non-refundable $25 fee.
