    Fill out this form to PREQUALIFY you for an interview with Belinda. This is only the first step in our adoption process.  This is NOT the adoption application. We do not keep previous questionnaires on file. We require a new one for each dog you are interested in. 

    When choosing a dog, it’s important to remember that the “right” dog for your family isn’t always the one you first had in mind.

    There are many factors to consider:
     ​- The dog’s energy and activity level
     - Your family’s lifestyle and activity level
     - Your experience with dogs
     - Whether there are children, cats, or other dogs in the home
     - The dog’s individual personality, which often matters more than breed

    While breed can provide some general guidance, at CB Rescue we’ve learned that every dog is unique. Behavior and temperament vary from dog to dog, regardless of breed.

    Belinda takes time to carefully review the information you provide about your family and circumstances. Based on her experience, she may recommend a different dog than the one you originally inquired about. This isn’t to discourage you—it’s to give both you and the dog the best chance at a happy, lasting match.

    Above all, please remember: never choose a dog based on looks alone. Finding the right companion goes much deeper than appearance—it’s about fit, trust, and love.

    We will only contact those for a phone interview who are the right fit for each dog.

    Adoptions fees are: $850 for dogs under 8 years old and $550 for dogs over 8 years old. 
    If you rent or live in a townhome or condo, you are required to provide proper documentation and agreement from your condo board that confirm the type of dog you are applying for is allowed on your property.
    By submitting this pre-qualifying questionnaire you are confirming that you have reviewed the adoption fees and you are in a financial position to complete the adoption should you be selected. CHECK YOUR JUNK MAIL FOR OUR REPLY.

    Important to know

    IF you are selected for a phone interview and there is a mutual agreement to move forward with the adoption application, it is important to know that there is a non-refundable $25 fee.
